What you get
Why it matters
Medicines often change in hospital: a new one started, an old one stopped, a dose altered. Mistakes in the weeks after discharge are one of the commonest reasons people end up back in hospital. A 20-minute check catches them.
What we do
We compare your discharge letter with what you took before and what your GP has on file, explain every change in plain words, and check you have what you need at home.
What you get
A clear list of what to take and when, any problems raised with your GP for you, and a pharmacist who knows your medicines for next time.
Quick answers
- Does the hospital have to refer me?
- Hospitals often send your discharge details to your chosen pharmacy, and we’ll check whether yours has. If not, you can still book: bring your discharge letter or medicines list.
- Can a family member or carer come, or book for me?
- Yes. Book in the patient’s name and say who’s coming. A carer can bring the medicines and the letter.
- What should I bring?
- Your discharge letter, every medicine you have at home (old and new), and your repeat prescription slip if you have one.
